FRANKLIN, Tenn., Aug. 21, 2026 — Fast Pace Health, a leading provider of urgent care, primary care, and multi-specialty healthcare services, continues it expansive partnership, in all markets it serves, with Upperline Health to offer the “Upperline Plus” coordinated care program to its Medicare patients age 65 and older.
The program is designed to strengthen the care patients already receive at Fast Pace Health by adding a layer of coordinated support services. Joining carries little to no additional cost depending on a patient’s coverage, and Medicare benefits, provider relationships, and existing insurance remain exactly the same.
Through Upperline Plus, eligible patients gain access to:
- 24/7 telehealth and on-call support for health questions and concerns
- A dedicated Care Navigator to help schedule appointments and connect patients to resources
- Remote monitoring tools to help track key health indicators from home
- Expanded wellness support, including pharmacist medication reviews and refills, dietician services, and wound-care resources
- Connections to community resources for food, housing, transportation, and mental health support with Fast Pace Behavioral Health services
Patients keep all of their existing insurance, specialists, and coverage. There are no restrictions on the providers they see, and their day-to-day care at Fast Pace Health stays the same.
